New Patients: Please plan to arrive 15 minutes early for your first visit to Lifetime Dental Care in order to complete the brief medical history forms and your dental insurance information, or you may print them from the links below and bring the completed forms to your first appointment:
If you have recent dental x-rays (obtained within the last 12 months), please bring them to your initial appointment. If you are being referred to Lifetime Dental Care from another dentist, specialist, or patient, please let us know who referred you to our practice. We like to thank those who refer others to our practice.
While we participate with Delta Dental (Traditional plan) and Blue Cross Blue Shield (Traditional plan), we are happy to bill your insurance company for your dental procedures even though we don’t participate with your insurance plan.

For your convenience we accept Visa, MasterCard, Discover and American Express. Payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, simply contact us.
Our office also offer affordable financing through Care Credit. Financing plans are available for people with a wide range of credit history. There are no fees to apply. There are no prepayment penalties. You may apply online, by phone or simply come in to our office and we will help you complete the necessary application. Based on your treatment needs, you may be eligible for up to 12 months no interest.
As a courtesy, our office will submit claims on behalf of patients with dental insurance. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
